Create a Supplier Document Review Form

A guide to create a form to be used to review supplier documents (such as SWMS & SDS)

To review supplier documents (such as SWMS, SDS, Insurances etc) you may want to have a checklist to appear when approving uploaded documents. This can help the approver ensure that the document is meeting company and compliance standards.

Requirements

Permissions: Company/Site Hosts permissions. If you do not have these permissions, please reach out to your 1Breadcrumb administrator.

Note: Make sure that you already added your questions or information in the Questions Repository before creating the Form. See how here

To add a PDF, Video or document, you must have added it as an announcement first. See how here.

 


Web Portal Instructions 💻

Step 1:

  • Click on Editor in the left hand side menu
  • Click on Forms in the drop down 
  • Click the blue New Form button on the right hand side to bring up a pop-up

Step 2:

  • Set the Type as Supplier Document Review.
  • Then, use the add question button to add questions, or add documents or add announcements.
  • Name the Form
  • If Multiple documents are collected select which ones you want this form applied to under Supplier Document Kind(s)
  • Once all information has been added in, click the blue add button.

This function is not accessible on the mobile app. Please log in to your 1Breadcrumb web portal to view the form report.
