Create Question Sets

1Breadcrumb's customisable Questions tool adds a personal touch to your Inductions, Permits, Forms, and Check-In/Outs, making these features gather the data that fits your business needs.

Web Portal Steps 💻

  1. Navigate to the 'Editor' tool located on the left-hand toolbar of your 1Breadcrumb portal. Select ‘Questions’ from the dropdown menu. A list of questions created will appear. You can delete irrelevant or obsolete question sets, however, any question sets linked to inductions, SWMS review checklists, forms, etc., cannot be deleted.

  2. Click the green '+ Add New Question' toggle.

  3. In the pop-up window, complete the Question Set configuration setting fields. (Click Here for a Description of each Question Set Configuration Settings)

  4. Upon making all the necessary changes, click the ‘Submit’ button in the bottom right hand side of the pop-up window.

Description of Question Set Configuration Settings ❓

  1. ‘Question' field: type the specific question for individuals to answer.
  2. ‘Use Rich Text Title’ checkbox: if you want to provide additional context or use bullet points to explain the question.
  3. ‘Question Type’
    1. Yes/No: For simple "Yes" or "No" responses.
    2. Options - Single Select: This question type, respondents can choose one option from a list of provided choices. It's suitable when you want users to pick a single response from a set of mutually exclusive options.
    3. Options - Multi-Select: Similar to the single select, this question type lets respondents select multiple options from a list. It's useful when you want users to choose more than one response from a list of options. 
    4. Text: This question type allows respondents to input free-form text as their answer. 
    5. File: Respondents can upload a file as their response. This is suitable when you need users to provide documents, images, or other files as part of their answer.
    6. Static Text: No response needed.
    7. Date: This question type enables respondents to pick a date from a calendar interface. 
    8. Signature Pad: This question type allows respondents to provide a digital signature. It's commonly used when you require a formal acknowledgment or agreement.
    9. Asset Field: Asset fields allow respondents to provide details or information about a particular asset. 
    10. Profile Info: This question type is often used to capture and display respondent profile information, such as name, contact details, job title, and other relevant data.
  4. ‘Question Is Active’ toggle: Inactive questions won't appear in future inductions, forms or check-ins/outs.
  5. ‘Ask Question’ dropdown list: Select the frequency you wish the question to be asked by choosing from the options below. Note: this setting will only have an impact if the question is configured as a check-in/out question:
    1. Always: If you select "Always," the question will be asked each time an individual checks in or out, without exception. Regardless of how many times the individual checks in or out in a day, this question will appear every time.
    2. Only one time: Choosing "Only one time" means that the question will be asked only on the first check-in or out, and it won't appear during subsequent check-ins or outs.
    3. One time per site: When you select "One time per site," the question is asked just once for each specific site, regardless of how many times the individual checks in or out from that site. If you have multiple locations or sites, the question will repeat once for each site.
    4. One time per day: With "One time per day," the question will be presented to the individual once every day, irrespective of the number of check-ins or outs that occur on that day.
    5. One time per day per site: This option combines the concepts of "One time per day" and "One time per site." The question will appear once every day for each site, even if there are multiple check-ins throughout the day.
  6. ‘Ask Mode’: Note: this setting will only have an impact if the question is configured as a check-in/out question:
    1. Anytime: Selecting "Anytime" means the question can be asked at any moment, regardless of whether it's tied to a specific action like check-in or not. It allows for flexibility in when the question is presented.
    2. Check-in: When "Check-in" is chosen, the question will only be asked when an individual checks in. It won't be presented during check-out or any other action.
    3. Pre Check-in: "Pre Check-in" refers to asking the question before the check-in process. This mode allows individuals to answer the question even before they physically arrive at the site for check-in. It offers the option to prepare for check-in before actually being on-site.
  7. 'Question Default Mode': (It is advised to keep this setting as 'Disabled' since 1Breadcrumb will soon phase out this field, making it unavailable for selection).
  8. ‘Question Is Sensitive’ toggle: Make question viewable by specific admins only.
  9. ‘Is Required?’ toggle: This is where you set the question to be mandatory or not to complete whatever induction/form it is used in. If set to Yes, it will ensure a user has answered the question before allowing them to continue.

Requirements ❗

  • Only Company Hosts and Site Hosts with Question permissions can create a question set. If you do not have permissions to create or edit a question, please reach out to your Company and Site Hosts. 
  • This function cannot be performed via the Mobile App. Please log into 1Breadcrumb via your computer or laptop.
