Create a SWMS/RAMS Review Checklist

This feature allows you to create a form as a Supplier Document Review (i.e. SWMS review checklist).

Before you are able to set the Supplier Document Review, you need to create a new Form or you may edit an existing form. Make sure that you already added your questions or information in the Questions Repository before creating the Form.

To create a new form, please see the article "How to create a Form?".


For New Forms, make sure that the Form Type is set to Supplier Document Review.

Fill in the other details of the form and click .


For Existing Forms, pick the Form you wish to edit and click .

Similar to the sample above, make sure that the Form Type is set to Supplier Document Review.

Once you're done, click .
